Why We Exist
Good medicine shouldn't require a waitlist, a long drive, or a referral that goes nowhere.
That's the short version. The longer one is below — but it starts and ends in the same place: people who need help should be able to get it without the system getting in the way.
Our Mission
To provide psychiatric and primary care that's honest, accessible, and actually built around the person asking for it.
When Dr. Thaker started Wexford Health Solutions, the goal wasn't to build something particularly ambitious. It was to build something functional — which, in healthcare, turns out to be the harder problem. A lot of the system is technically available and practically inaccessible at the same time. Psychiatrists with six-month waits. Practices that take your insurance on paper and never have an opening. Rural counties where the nearest specialist is a hour and a half each way, and that's if the weather cooperates.
Wexford exists to solve a narrower version of that problem. Not all of it — one telehealth practice isn't going to fix a broken system — but the part of it that Dr. Thaker can actually address. Which is: getting people a real appointment with a clinician who knows their case, without the barriers that usually stand between that person and that appointment.
What that looks like in practice is a deliberately small patient panel, so no one waits months to be seen. Telehealth visits, because the drive to an office shouldn't be the reason someone delays care. And one doctor, consistent, visit after visit, because continuity isn't a luxury — it's the part that actually makes treatment work.
"I wanted to practice medicine the way I'd want to experience it if the roles were reversed. A doctor who knows you. A visit that isn't rushed. A plan you helped make."
— Dr. Nilay Thaker, D.O., Founder
Mental health is part of that, but so is the rest of the picture. Anxiety that's tangled up with a blood pressure issue. Depression that tracks with chronically poor sleep. ADHD that's made worse by untreated stress. Dr. Thaker trained as a Family Physician for exactly this reason — because treating one piece while ignoring the others often means you're not actually treating the problem.
That's the practice. That's the mission. The rest is details.
What this actually looks like
Mission isn't a paragraph on a website. It's what happens at 3pm on a Tuesday.
Prompt appointments
A small patient panel isn't a capacity limitation — it's a decision. Most new patients are seen within the same week.
One doctor
Dr. Thaker sees you. Not a rotating associate, not a different provider every few months. The same person, every time.
No geography
Licensed across NY, PA, MD, and WV. Whether you're in a city or two counties from the nearest clinic, the visit is the same.
Whole picture
Mental health and primary care in the same practice, because most people's health doesn't respect specialty silos.
