1. Accurate Completion of Initial Intake Form:
An accurate, complete, and truthful initial intake form must be signed and submitted before your appointment. Omitting essential information or providing false information on this form may result in dismissal from WHS
2. Fee Structure:
Initial visits are $175, and follow-up visits are $115. WHS operates as a concierge family practice and does not accept insurance. In the rare instance where deferred payment is offered, patients will be unable to schedule another appointment until the outstanding balance is fully paid.
3. Payment Methods:
Wexford Health Solutions uses Square Payments, a fully secure and HIPAA-compliant system that accepts credit and debit cards. Full payment is required at the time of booking an appointment
4. Appointment Requests:
It is the patient’s responsibility to utilize the practice website to schedule
appointments in a timely manner, ensuring continuity of care. We recommend booking follow-up appointments at least one week in advance. Failure to plan accordingly does not constitute an emergency, and our schedule cannot be adjusted to accommodate for this.
5. Reschedule/Cancellation Requests: Appointments can be rescheduled or canceled by clicking the reschedule/cancel link at the bottom of your Square confirmation email or by contacting the practice via email.
6. No-Show Policy:
Respect for both your time and ours is essential. Failure to attend an appointment without prior notification will incur a $50 no-show fee. Repeated no-shows will result in discharge from WHS
7. Work or School Excuse Notes:
If you require a formal excuse letter for medical necessity, please request it during your visit, or send an email to the practice. Kindly allow a turnaround time of 72 hrs. The fee for this service is $25.
8. Appointment booking:
Patients should book appointments under their own name, address, and contact information, and not under that of a family member.
Medication Management:
We require a comprehensive medication list to be reviewed and updated annually or as necessary.
Controlled Substance Agreement:
Reasons for potential discharge from WHS include, but are not limited to:
1. Seeking controlled substances from multiple healthcare providers.
2. Abuse of staff.
3. Failure to follow up on care in a timely manner.
4. Providing false information.
5. Repeated requests for early refills or increases in dosage.
6. Failure to comply with the terms set out within this document.
Discharge Procedure:
1. A final prescription of 30 days will be provided for the patient while they find a new clinician.
2. Patients will be notified via SMS and email, which will include an official discharge letter.
3. Wexford Health Solutions will not provide care beyond this 30-day period.
