Appointments can be booked via the main page in the, 'Online Appointments', section.


Appointments can be amended by clicking the, 'reschedule or cancel', button at the bottom of your confirmation email from Square. Alternatively, you can email us at help@wexfordhealthsolutions.org.




To cancel an appointment, you can do so by clicking the, 'reschedule or cancel', button at the bottom of your confirmation email from Square. Please leave at least 24 hours before cancelling your appointment. Failure to to do so will result in a $50 fee.

Telehealth consultations will be conducted via a telephone call at the scheduled time of your booked appointment. You can find your appointment time in your booking confirmation email. 


Dr. Nilay Thaker, DO, specializes in Family Medicine and Mental Health. He treats a wide range of conditions, including anxiety, depression, ADHD, and insomnia, as well as chronic medical issues such as hypertension, diabetes, and migraines, among others.

 

If you are experiencing a medical emergency, please call 911 or go to the nearest emergency room.


For all physician appointments, you will be seen by Dr. Nilay Thaker, DO or our Physician Assistant.  Nursing appointments, including MAT and counseling sessions, will be managed by Sarah Cooper, RN, BSN.


Wexford Health Solutions, LLC, does not accept insurance, as we are a concierge medical practice. 


For Physician appointments, Initial consultations are $175, which includes a 30 minute telehealth appointment with Dr. Thaker. Follow-up consultations are $115, and last for 20 minutes. Payment is due in full at time of booking an appointment.


Nursing appointments vary by price depending on the chosen service. 


Wexford Health Solutions, LLC does not guarantee that our clinicians will issue a prescription for medication. Prescriptions are based on what is medically appropriate for each patient on an individual basis. 


As a general policy, our practice does not prescribe opioid medications for pain management except in rare circumstances.


 

Wexford Health Solutions, LLC primarily accepts patients from New York, Pennsylvania, and West Virginia.
